MetLife Dental Product Video Overview

• District 62 has 1 dental plan through Metlife.  It is a PPO plan. 
• You can choose a dental provider in or out of the Metlife network.  If you choose an out of network provider you will generally pay more out of pocket for the services because the provider will not have a contract with Metlife to specify cost limits on services.
• Metlife does NOT send physical insurance cards.  To access your digital ID card to view and/or print it, please log into your account at metlife.com/mybenefits.  If you do not already have an account, you will need to register for one. When you register for the account and are asked for the organization name please use Community Consolidated School District #62.

• The PPO 500 and PPO 750 medical plans include vision coverage.  BCBS uses the EyeMed network of providers but the vision coverage is through BCBS, not EyeMed.  
  • The BCBS PPO 500 and PPO 750 medical plans provide a $30 allowance toward an annual vision exam for you and your covered family members, as well as a $200 allowance for materials (glasses, contacts, etc.) every 12 months (the 12 month time period is a rolling calendar year, based on the date of your last vision claim/visit).  You do NOT have to go to an EyeMed provider in order to receive the $30 exam and $200 materials reimbursements.
  • Payment for the exam and materials must be made out of pocket and then submitted to BCBS for reimbursement.  In order to obtain your reimbursement, submit an itemized receipt along with a medical claim form directly to Blue Cross Blue Shield.  The medical claim form can be found here:   https://www.bcbsil.com/PDF/formfinder/medical-claim-il.pdf.  
  • The best way to submit a claim is to log into your Blue Access for Members (BAM) account on the BCBS website (www.bcbsil.com).  
    • Go to messages (top right hand corner of the screen).  
    • Click New Message and type your reimbursement message in the body of the email and attach the claim form and the itemized receipt.  
    • Submitting the claim electronically will allow you to track the reimbursement.

The BCBS PPO 500 and PPO 750 plans also offer the Blue 365 EyeMed Vision Discount Program. 

The Blue365 program is NOT insurance.  It is a discount program only.  If your provider participates in the EyeMed network, show your BCBS medical card and mention the Blue 365 discount program to be eligible for savings on eyeglasses, contact lenses, eye exams, accessories, and laser vision correction.  Visit eyemedexchange.com/blue365 for more information or to find an EyeMed provider.  To locate a provider, click on Find a Provider to begin your search.  Be sure the Advantage network is selected. 

The HMO medical plan includes vision coverage.  BCBS uses the EyeMed network of providers but the vision coverage is through BCBS, not EyeMed. 

You MUST use the EyeMed network of providers in order to receive your vision benefits under the HMO plan.  This is just like using your primary care physician and medical group for your medical visits.  Employees and covered dependents enrolled in the District’s HMO plan are entitled to an annual eye exam at no additional cost and $150 towards contact lenses and/or $225 towards vision materials every 24 months.  To locate a vision care provider, please call EyeMed Vision Care at 844-684-2254.  To use your vision benefit, please show your HMO medical card at the time of your vision care visit.

The HDHP 1500 medical plan does NOT include vision coverage.  

If you elected the HDHP 1500 medical plan and you elected the separate vision plan, then your vision provider is VSP.  The VSP vision care summary of coverage can be found on page 31 of the Benefit Summary Guide available in the Benefit Hub here:  https://ccsd62.benefithub.com/.  VSP does NOT send physical insurance cards.  When you visit your vision care provider, simply tell them that you are a VSP member.
